The interpreter dials into the same line, and once everyone is connected can add a third person, such as a parent or a spouse. Nobody can see who is speaking, so each person waits until the other has finished.

The client's words reach the therapist in English, in the first person and in full, hard words included, and the therapist's words reach the client in the client's language.
